The Alachua County Board of County Commissioners is seeking proposals from governmental and non-profit 501(c)(3) agencies with a physical address in Alachua County for delivery of programs to aid in the reduction of health disparities and lack of access to care in the categories of medical services, oral health, and mental/behavioral health. Non-profit agencies must have a current IRS 501(c)(3) designation.
All CHOICES applications must be completed electronically via ZoomGrants beginning 12:00 pm, June 12, 2019 and submitted no later than 11:59 PM, July 10, 2019.

No late proposals will be accepted. Proposals that do not meet the grant criteria will be rejected. Application rules and instructions, including the complete RFA document, are available on the site and interested vendors are encouraged to read all related documents prior to beginning an application.
The CHOICES grant is a reimbursement grant, requiring grantees to expend their own funds and request reimbursement. The grant period is from October 1, 2019 through September 30, 2022, subject to annual appropriation by the Alachua County Board of County Commissioners.
