Starting the week of August 1, 2011, the hours will be:

Monday  8:30 a.m. - 5 p.m.
Tuesday  8:30 a.m. - 6 p.m.
Wednesday  7:30 a.m. - 5 p.m.
Thursday  8:30 a.m. - 6 p.m.
Friday  8:30 a.m. - 5 p.m.

CHOICES is a health services program offered by the Alachua County Board of Commissioners. The program is designed to help uninsured residents of Alachua County stay healthy. Through partnerships with local medical and dental care providers, CHOICES provides access to services that many working, uninsured people need but may put off due to a lack of adequate health coverage.

For more information about the Alachua County CHOICES Program click here, or call 352-334-8889.

If you have a disability and need an accommodation in order to participate in a County program, service or public meeting, please contact the Alachua County Equal Opportunity Office at (352)374-5275 at least 3 business days prior to the event. TTY users please call 711 (Florida Relay Service).
