Birth Certificates Available at Drivers License Office

This release sent at the request of the Alachua County Health Department.

ALACHUA COUNTY - Effective January 1, 2010, residents applying for a State of Florida drivers' license or I.D. card have to comply with new identification requirements.  For office visits, you must bring original documents that prove your identity, social security number and residential address.  One of the accepted identification documents is a Certified United States Birth Certificate.  The Alachua County Health Department (ACHD) has partnered with the Department of Motor Vehicles to provide certified birth certificates at the drivers' license office located at 5830 NW 34th Street, Gainesville, phone number 352-955-2111.

"We are pleased to provide this service to Alachua County residents." stated Jean Munden, Administrator of the ACHD.  "Being able to issue a certified birth certificate at the driver's license office is convenient and efficient."

Birth certificates cost $15 for a certified copy and $7 for each additional copy.  The Alachua County Health Department, Office of Vital Statistics, can access birth records for anyone born within the State of Florida from 1930 to the present.

Any individual 18 years of age and older may obtain their own birth certificates and parents or guardians (who present court documents showing legal custody) can obtain one for their dependants.

In order to obtain a birth certificate, please provide the following:

  • A valid form of photo identification such as a driver's license, State ID issued by the Department of Motor Vehicles, passport or military ID.
  • A completed Birth Certificate Request Form

You will also need to provide the person's:

  • Full name
  • Date of birth
  • County of birth
  • Mother's full name including maiden name (for Birth Certificates)
  • Father's full name (if applicable on Birth Certificates)
