Refer Patients to CHOICES

Providers are encouraged to refer patients who may be eligible for CHOICES. Applying for CHOICES is part of a comprehensive screening and may include enrollment in other services that the County offers. Applications are regularly distributed to providers and should be given to potential Enrollees.

If you need more application packets, call CHOICES at (352) 264-6772. We will mail or deliver them to your office. Your patients may download the application packet.
