  1. How to I become a Provider?
    Please call BCBS at 1-800-727-2227 so they can assist you with questions and concerns.
  2. I am a Primary Care Physician.  Do I need to do a referral for a CHOICES member?
    No, CHOICES members will not need referrals.
  3. I am a Blue Care (HMO) Specialist.  May I see a CHOICES member without a referral?
    Yes you may.  CHOICES members will no longer need referrals from their PCPS effective 10/01/10.
  4. Who do I contact with questions regarding eligibility and coverage?
    You would contact BCBS.  Information can be found at Availity as well.
  5. Who do I contact if my claim was denied but the service was a covered service?
    You would contact BCBS.
  6. I am a BCBS Blue Care HMO provider and am interested in a CHOICES overview for my staff?  Who do I contact for this?
    Please contact CHOICES Health Services at 352-264-6772 to speak with a member of the Provider Relations Team to schedule this presentation.
  7. I am a BCBS Blue Care HMO provider.  If one of your CHOICES members presents to my office for an appointment, who do I call?
    Please contact BCBS if you have questions regarding eligibility or services or you may view this information through Availity.
  8. What if I have self-pay patients who fit the criteria for CHOICES?  Who would I contact?
    Please contact the local CHOICES Office at 352-264-6772, so that we may reach out to this potential member.
