CHOICES Health Services provides enrollees with:

• the primary and preventive care services of a family doctor or nurse practitioner
• medically necessary specialty care
• dental care services
• vision care and prescription eyeglasses
• assistance with prescription medicines
• disease management services for chronic illnesses
• health education and wellness classes, support groups and information
