To enroll in the CHOICES Health Services Program, applicants must meet the following eligibility criteria:

  • Age 18 – 64*
  • U.S. Citizen or permanent resident
  • Alachua County resident
  • Employed an average of 80 hours monthly
  • Not eligible for Medicaid or Veterans benefits
  • No health care coverage or unaffordable employer health benefits (affordability criteria apply)

*For family units of more than 8 members, add $7,920.00 annually; $660.00 monthly, for each additional person.

*Seniors age 65 and older may be eligible for dental care.
