CHOICES Disease Management Program provides assistance to CHOICES members with chronic health conditions such as asthma, diabetes, hypertension and coronary heart disease. For many people, chronic conditions result in continued pain and suffering and decreased quality of life. However, properly managing chronic conditions can reduce suffering and help people live longer.

Enrollment in CHOICES Disease Management Program can be a critical step in receiving the tools needed to manage chronic conditions.

CHOICES Disease Management Program will provide CHOICES enrollees with the following:

  • A registered nurse who works with members and helps them manage their conditions.
  • Locating and coordinating resources in Alachua County that make it easier for members to improve health conditions.
  • Easy to understand information about chronic conditions.
  • Assistance with treatment plans provided by primary care providers.
