There are several ways to apply for the CHOICES Health Services Program. To accommodate the lifestyles of working individuals, CHOICES has created an application process that allows county residents to apply for the program on-site, off-site and before and after office hours. You can choose the option that best suits your needs.


Enrollment Options

Option 1:
Come to Open Enrollment


Office Hours:

8:30 am - 5:00 pm

Wednesdays:

7:30 am - 8:30 am

Complete and Print an Application

Applicant Must Provide:

  • Photo ID
  • Proof of Citizenship/Legal Residency
  • Proof of Alachua County Residency
  • Proof of Employment and Income (form is included in Application Packet)
  • Proof of Spouse Employment and Income (form is included in Application Packet)
  • Proof of Self Employment and Income (form is included in Application Packet)
  • Release of Information (form is included in Application Packet)
